Abstract

Introduction: Acute respiratory distress syndrome (ARDS) is a severe medical condition, and mechanical ventilation is a supportive therapy in this situation. The high-frequency oscillatory ventilation (HFOV) may be considered an ideal lung protective ventilation strategy; however, some studies indicate that may cause hemodynamic deterioration. Objective: To evaluate the hemodynamic effects of HFOV. Method: Case report of a girl of 1 year of age with ARDS unresponsive to conventional treatment with evolving hemodynamic deterioration. Results: After HFOV there was in improvement in hemodynamic variables, weaning of vasoactive drugs and increase on diuresis. The oxygenation index (OI) dropped more than 50% in the first 24 hours. HFOV was used for 11 days with a return to conventional ventilation in minimum parameters. Conclusion: HFOV can be used as rescue therapy even if the hemodynamic conditions are not favorable in patients with severe ARDS.
